Many of the LODDs occur as a result of a combination of failures involving basic firefighting tasks, initial implementation of standard firefighting strategies and tactics, and the inability to perform firefighting skills in zero visibility.

Additionally, the authors illustrate, on the drill ground, concrete tactical drills to help combat and minimize these errors on the fireground.
